MediGold's Quality Care Connection provides many valuable resources to help you live your best life, including our Home Care Transitions Program, personalized outreach calls to remind you about preventive services, access to a free Healthy House Call, health tips and expert guidance provided by our experienced team of MediGold nurses.
MediGold Care Management
If you're interested in personal medical assistance related to a disease or chronic condition, our nurse care managers can provide you with free and confidential guidance over the phone. Support includes:
- Information to better understand and manage your condition or disease;
- Personalized answers to your health or medication questions;
- Coordinating multiple services such as homecare, medical supplies or medical equipment; and
- Advice on how to live safely at home.

Healthy House Calls
A Healthy House Call is a free service offered to all MediGold members. It takes place in the comfort of your home or any location convenient for you. A nurse practitioner will perform a physical assessment and share the results with your primary care provider. To schedule your Healthy House Call, please call 1-866-621-6254 (TTY 711), 8 a.m. – 5 p.m., Monday through Friday.
